Eligibility Form
Required by Bellevue School District for all band students.
  • States rules for eligiblity.
  • Requires signatures of Student and Parent.
Health Form
Required by Newport High School for all band students.
  • Provides personal insurance and health information required for one-day field trips.
  • Overnight or longer trips require additional forms.
  • Requires signature of Parent.
  • Please submit updated form whenever changes occurs.
Substance Abuse Form
Required by Bellevue School District for all band students.
  • States substance use rules, and penalties for non-conformance.
  • Requires signatures of Student and Parent.
